Medications at school and health treatment plan for 2026-2027

Please have all health treatment plans, medication administration forms, and brand-new, unopened medication for the upcoming 2026–2027 school year signed and ready to be handed in beginning August 17, 2026. Please refer to the Nurse page "Health treatment Plans" to obtain all treatment plans and forms or visit PWCS Student Health Services website.

Important Guidelines

  • School personnel cannot administer any medication until a form, signed by the parent/guardian is on file in the clinic of the school. All prescription medication must also have a physician's signature on the form.
  • Any medication, prescription, and over-the-counter must be provided in a new, sealed container with the completed authorization form.
  • An adult needs to bring to the clinic any and all medication. No medication is to be sent to school with the student.
